Care Manager Job Description: Full Time, Non-Exempt
Organizational Background: The SCV Senior Center a 40 – year old non-profit organization serves over 10,000 seniors each year. We focus on nutrition, support, recreation, day services and much more.
Position: The Case Manager provides comprehensive care management by assessing and arranging appropriate services to older and disabled individuals.
Qualification: Minimum of a BA degree in Social Services, Gerontology, Nursing, Other Health Related Fields, 2 years – experience in in the aging or health related field, and non-profit experience.
Excellent written and verbal communication skills. Experience working with multiple staff members. Proficiency in Microsoft Word, Windows, and database applications. Able to work independently with little supervision. Professional appearance and telephone presence. Problem-solving skills and good judgment. Well-organized, self-motivated and ability to multi-task. California driver license, proof of auto insurance, reliable transportation. Willing to travel within the Santa Clarita and Antelope Valley. Ability to initiate and be self-starter, excellent supervisorial skills, team player and strong interest in the mission of serving seniors.
Compensation: Compensation DOE. This is a full time, exempt position with a competitive benefits package.
